You are invited to take part in a research study being conducted and funded by the Hearing Health Alliance of Canada. The purpose of this research is to gain insights about the experiences of people with hearing health issues, which may include hearing loss.

The survey should take approximately 5 to 10 minutes.

Your participation in this research is entirely your choice. You do not have to answer questions that you do not want to answer, and you are welcome to stop the survey at any time if you no longer want to participate. All you need to do is close your browser. We anticipate that there will be no risk of harm to you, however some of the questions are asking about experience with hearing loss and you or your dependent might experience some feelings related to those issues. Remember you can stop at any time and are not required to answer all questions.

Your responses to the survey will be anonymous. This means that there are no questions in the survey that ask for identifying details such as your name or email address. The survey is created in a survey tool which has no method of tracking your identification or location. Your health care practitioner and the study team will not know if you have answered the survey. Most of the questions ask you to select a reply from a list of options. If you choose to provide comments within the survey, some of your reply comments might be used in reports and will never be linked to your name or other identifying factors. This survey has been approved by Dalhousie University Ethics Review (Dal REB file number 2020-5043 ethics@dal.ca).
